OptumRx, under UnitedHealth Group, is one of the largest Pharmacy Benefit Managers (PBMs) and mail-order pharmacies in the United States. Amazon is attempting to disrupt the traditional pharmaceutical supply chain from the edges by acquiring PillPack and launching services like Amazon Pharmacy and RxPass. Leveraging its powerful logistics network and vast Prime membership base, Amazon has launched highly attractive services (such as RxPass, which offers unlimited generic drug subscriptions for $5 per month, and two-hour delivery). This directly competes with and has taken market share from OptumRx.
Amazon's strategy is to make drug prices completely transparent and e-commerce-driven. When consumers find that buying drugs directly from Amazon in full, even without using UnitedHealth's insurance, is cheaper than going through insurance, the rationale behind the traditional PBM model is seriously challenged. This trend towards transparency not only squeezes OptumRx's profit margins but also brings more regulatory and antitrust scrutiny pressure to the PBM industry.
