From a professional pilot's perspective

It's unlikely to be a flap issue

Before take off check list has an item for flaps

Even if it's missed, there would be a warning if flaps aren't deployed during takeoff when thrust N1 is applied

Even if that's also missed, if the stall warning sounds, the pilot can still recover by pushing TOGA

I think it's more likely a TOGA or N1 setting error

If there are no flaps

We have a checklist before takeoff

Even if the checklist is missed

There would be a very loud warning when applying takeoff thrust

Even if not heard, using full power (usually not used during takeoff to extend engine life) can still 'get airborne'

So I think it's a thrust setting error
